Cardiac Medical Services, Inc. bills per patient on a HCFA 1500 form. 
  • The average charge is 80% of Medicare.
  • Charges are subject to change based on volume.

Prior to any test, CMS needs the following information:

  • Face sheet or patient information sheet.
  • Authorization with ICD-9 and CPT codes.
  • Doctor's orders with diagnosis ICD-9 code has to correspond with CPT code of the test ordered.
